HAZARDOUS MATERIAL: Department of Transportation has determined that certain products require special handling. $35.00 per box will be charged to cover additional handling. Some hazardous product may be restricted from shipping via AIR. There may be certain products that cannot be shipped to the following locations: PR, VI, HI, AK, and Guam.

ACCOUNT: 30 days net for Schools using a Purchase Order (some restrictions apply), a 1.5% Service Charge will be added to past due accounts (compounded every 30 days). Net 30 days with approved Credit. Price F.O.B. - shipping point. See the back of Pocket Nurse "Invoice" for Terms and Conditions.

Minimum orders: Merchandise total under $25.00 will include a $10.00 Service Charge.

Order Changes: Once an order is processed, it CANNOT be changed. Please submit a new Purchase Order to your Account Manager.

Back Order Policy: All backordered products will ship immediately upon receipt from our supplier. Pocket Nurse will make every effort to notify you should the status of your order extend past 30 days, giving you the option of canceling the item or accepting an available replacement.

Sales Tax: All businesses and individuals domiciled in Pennsylvania will be charged 6% sales tax unless order is accompanied by a copy of your tax-exemption certificate. All others please refer to your specific State's Use Tax Code. PST/GST and brokerage fees will be added if applicable.

Product Specifications: Pocket Nurse Enterprises, Inc. reserves the right to change product specifications on all generic items without notice.

Damaged Shipments: Any visible damage, however slight should be notated on the freight bill at time of delivery. No notation on the freight bill states you received the merchandise in good condition. If visible damage is noted, you may request the driver to do an immediate inspection. Please refer to the packing slip enclosed with your order for additional information.

Missing Merchandise: Please report all missing merchandise to your Account Manager, within 10 days of the invoice date.
Occasionally cartons separate during shipment; however all cartons should arrive within 1-2 working days.

Returns: A return Merchandise Authorization (RMA) number MUST accompany all returns. Should you need to make a return, please contact your Account Manager within 30 days of receiving the product. Keep in mind, merchandise MUST be returned within 30 days of the RMA number being issued. Merchandise should be securely wrapped and returned in its' ORIGINAL CARTON and the RMA number must be visible on the outside of the carton. MERCHANDISE MUST BE SENT TO YOUR RECEIVING DEPARTMENT. Returned merchandise will not be accepted after 30 days of the RMA issue date and will be returned to the sender. There is a 20% restocking fee on ALL returned goods. See "Return Goods Policy" on the back of Packing Slip.

Any defective product that is returned during the warranty period will be repaired or replaced with authorization and prepaid freight.
